This patient messaged me out of frustration the other day.

He wrote: “

Sir My hair transplant was done 4 years back. Two assistants extracted and punched grafts under doctor supervision.

After 1.5  year I found that many grafts lost from left  side of head which was operated by One assistant  

while right  side of head done by second assistant was perfect with no loss of grafts.

This shows experience and hands of assistants are also important.

 

You’re absolutely correct, and your experience highlights a crucial aspect of hair transplant surgery—technical skill and experience are essential at every step, including graft extraction and placement. 

A study in India revealed that over 97% Indian clinics are doing hair transplant without even the supervision of a doctor. These are teams that take an OR/OT on rent and do the procedure under the umbrella of a doctor who is well known in the area to instill confidence.

Over 75% clinics do not have regular staff and engage teams of technicians on case to case basis. So consistency is lost and so is the quality. The result in these clinics is not stndardised and will be as good as the team that came in that day.

As per regulations only the doctor who should be either a plastic surgeon or a dermatologist should perform all the esthetic & surgical steps- hairline design, anesthesia, extraction and slit making. Unfortunately less than 1% clinics follow this regulation.

In my clinic, the slits are filled by expert technicians who are on the clinic payroll and the youngest among them has been with me for 6 years. So attrition rate should be minimum as far as the staff is concerned for safety and quality.

Work done by assistants can significantly affect the outcome. Each step, from extraction to placement, requires meticulous attention to detail, precision, and experience, particularly with regard to factors like angle, depth, and handling of each graft. These factors can greatly influence both immediate graft survival and long-term hair growth.

In your case, the difference in outcomes between the left and right sides seems to underscore how variability in an assistant’s technique, skill, or experience can impact results. An experienced assistant is skilled in ensuring that grafts are handled gently and placed in a way that maximizes their survival rate.
